- Standard Enrollment Form
- A form that you can print out, complete and either fax or mail to us to complete your enrollment.
- Policy Agreement
- This is the current Policy Agreement for enrollment at The Music Place. It explains our policies and procedures about Payments and Credits, Notification of Absences, Makeup Lessons, etc. Feel free to ask questions or make comments.
- Online Payment Enrollment
- Use this form to signup for Automatic Tuition Payments. This allows your monthly tuition to be automatically deducted from your checking account at the beginning of every month... and it's free!
